Fill Out Registration Form and listed documents
Spaces are offered subject to availability. If there is no space available, you will be placed on the waitlist if you desire to wait or move your child later. Please print, fill out the Registration Form and email back. Registration is not complete, and care will not commence until all the paper work is done! Prior to the start date of care the following must be received by Laura Lee’s Little Learning Center for each child:
• Registration Form
• Immunization Record
• Parent/Caregiver Contract
• Non-Refundable Deposit
• Daycare Subsidy Request Form if applicable

To ensure that the programs we offer run as smoothly and as professionally as possible, Little Learning Center have policies in place to preserve the integrity of our programming, children, and staff.
Prior to enrollment, we encourage all registered and prospective families to review Policies, Parent/Agreement Contract and fill out all necessary documents and forms included in the Registration Package.
Email or Bring in Person
Email all completed forms to lauraleeneumann@hotmail.com or print them and bring in person. All documents have to be signed / initialled and returned to Laura Lee’s Little Learning Center prior to start date of care.
Policies & Procedures
PROBATIONARY PERIOD
The care provider and/or parent(s) reserve the right to request that a child is withdrawn from care if he/she is unable to adapt or adjust within the first month, or in the care providers’ opinion, has social, emotional, or physical needs that the care provider feels he/she is unable to provide for. During this period, only one week’s notice will be require by wither the caregiver or the parent(s). A portion of the fee paid will be refunded in this instance.
NOTICE OF WITHDRAWAL
Once the probationary period has passed both the Parent & Caregiver agree to give one month’s written notice on the first of the month, when the child(ren) is/are to be withdrawn from the child care. The Parent(s) will pay one month’s fee in lieu of notice.
GRADUAL ENTRY PERIOD
When a new child enters my care, it is good to do a gradual entry period to help them adjust to being separated from their home environment and their parents. I will ask the parents to stay with their child for a short time the first couple days. As the child begins to feel comfortable, the parent can stay away for longer periods of time. This time period may be different for all children depending on how they adjust. Usually we recommend no longer then one week. I will charge an hourly rate of $10 during this time.
TELEVISION USE
Television use will be limited to Friday afternoons or days when we are unable to go outside due to weather conditions. Sometimes I will have a show on for children for a 10 minute period before nap while I am cleaning up after lunch. We will watch no more than one hour of television during a day. Viewing will be limited to child-friendly, educational-style videos, such as Paw Patrol, Dora, Bubble Guppies, Sesame Street, etc. I carefully screen everything the children watch and watch along with the children. I avoid any type of graphic images, potentially frightening images, unpleasant language, or anything of that nature, as we believe that young children are extremely impressionable.
REGISTRATION COMPLETION
Registration is not complete, and care will not commence until all the paper work is done! Prior to the start date of care the following must be received by Laura Lee’s Little Learning Center for each child:
• Registration Form
• Immunization Record
• Parent/Caregiver Contract
• Non-Refundable Deposit
• Daycare Subsidy Request Form (if applicable – parent is responsible for picking up this form at the Daycare Subsidy Office for Child’s Play Family Daycare to sign. Parent is responsible for making sure that the form is received by the Daycare Subsidy Office IMMEDIATELY UPON SIGNING – Care will not commence until we have confirmation of acceptance from the Daycare Subsidy Office)
TERMINATION
Laura Lee’s Little Learning Center reserves the right to terminate a child for the following reasons (but not limited to):
• Failure to pay
• Routinely late picking up your child
• Failure to complete the required forms
• Lack of parental cooperation
• Failure of child to adjust to the center after a reasonable amount of time
• Physical or verbal abuse of any person or property
• Our inability to meet the child’s needs
• Lack of compliance with handbook regulations
• Serious illness of child
I appreciate as much advance notice as possible when terminating, and will give the same courtesy in return. Parents are required to give three weeks written notice when they decide to terminate child care. The three weeks prior to the child’s last day must be paid in full, regardless of whether or not the child is in attendance.
I will give three weeks notice prior to termination as well for which full tuition is due, whether or not the child is in attendance. I reserve the right to give written notice of immediate termination where there are extreme circumstances that affect the well-being of the provider or other children in attendance.
Anyone who terminates daycare and has a balance that is outstanding will need to have the account settled within 30 days. All accounts not settled within 30 days will be turned over to a collections agency regardless of amount owed. All accounts turned over to collections will have a $20 per week late fee plus 35% collections fee added to amount due.
